Can you really catch an STI by wearing second-hand clothes?

Sexually transmitted infections (STIs) spread through direct contact with mucous membranes or biological fluids during unprotected sexual intercourse. This medical definition immediately excludes most scenarios involving a garment hanging on a rack in a thrift store.

Confusion persists because textiles can indeed harbor microorganisms. Therefore, the question deserves to be asked precisely: which pathogens survive on fabric, and which pose a real risk of textile transmission?

Survival of pathogens on textiles

A study published in 2024 in the Journal of Hospital Infection documented the persistence of certain bacteria on textile fibers for up to 206 days. This figure, reported by microbiologist Primrose Freestone from the University of Leicester, fuels concerns.

The details matter. The microorganisms identified in this study are primarily skin bacteria such as Staphylococcus aureus, dermatophyte fungi, and enteric viruses. None of these agents are classified as STIs.

The topic of the myth or reality of STIs and second-hand clothing rests on this distinction between skin microorganisms and sexually transmitted pathogens, two categories that the general public often conflates.

STIs and second-hand clothing: why transmission is biologically unlikely

The agents responsible for the main STIs (HIV, chlamydia, gonorrhea, syphilis, hepatitis B) share one characteristic: they are extremely fragile outside the human body. HIV becomes inactive within minutes when exposed to air, as soon as the fluid carrying it dries.

For transmission to occur, three conditions must be met simultaneously:

  • An infected biological fluid (blood, semen, vaginal secretions) still fresh and moist on the fabric
  • A sufficient quantity of viable pathogen, which implies very recent contact
  • An entry point into the body of the wearer of the garment, such as an open wound in direct contact with the contaminated area

This scenario does not correspond to any realistic situation of purchasing second-hand clothing. The time for transport, storage, and display is enough to render these pathogens non-viable.

The specific case of hepatitis B

Among STIs, the hepatitis B virus has the highest environmental resistance. It can remain infectious on a dry surface for several days under certain laboratory conditions. This case remains theoretical for second-hand clothing: no transmission of hepatitis B through textiles has been documented in the medical literature.

Non-sexual skin infections: the real risk of second-hand clothing

If STIs are not transmitted through textiles, other infections deserve attention. Scabies, caused by a microscopic mite, is the best-documented risk. The parasite Sarcoptes scabiei can survive for up to three days outside of human skin, nestled in the fibers of a garment.

Bedbugs represent another concrete problem. These insects can remain dormant for months in the seams and folds of textiles, including clothing stored in thrift bins.

A case reported in 2024-2025 highlighted molluscum contagiosum. A TikTok user developed skin lesions after wearing unwashed second-hand clothing. Molluscum contagiosum is a viral skin infection, not an STI, even though the virus can also be transmitted through sexual contact under other circumstances.

Dermatophytes and skin bacteria

The fungi responsible for ringworm or skin mycoses survive well on textile fibers. Staphylococcus aureus, a bacterium found on the skin of many people, can cause infections if it comes into contact with compromised skin or a wound.

These risks are real but manageable. They pertain to textile hygiene, not sexual health.

Washing before use: the gesture that neutralizes risks

A wash in a washing machine is enough to eliminate almost all pathogens present on second-hand textiles. Here are the parameters that ensure effective disinfection:

  • A wash at 60 °C eliminates scabies mites, fungi, and the majority of pathogenic bacteria
  • The addition of a textile disinfectant complements the action of temperature for delicate fabrics washed at low temperatures
  • Complete drying in a high-heat dryer destroys residual parasites, including bedbugs at all stages of development
  • Non-machine washable items (leather, certain coats) benefit from dry cleaning or a quarantine of several days in an airtight bag

A properly washed second-hand garment poses no more infectious risk than a new garment tried on in a fitting room by other customers. New clothes in stores are rarely sterile: they have been handled, tried on, and stored under varying conditions.

The second-hand market does not pose a sexual health problem. The confusion between STIs and common skin infections perpetuates unfounded medical concern, provided a simple gesture is respected: wash any second-hand clothing before wearing it. STI agents do not survive on dry fabric, and no clinical data document cases of transmission through this route.
